→Admission notification for Certificate in Community Health (CCH) for july 2022 batch

Instructions:

  1. Fill up the application form.
  2. Click the Next button to go to the Document Upload section.
  3. Upload all following documents:-
    1. Passport size Photograph (Mandatory)
    2. Signature (Mandatory)
    3. Age Proof Certificate (Mandatory)
    4. Final year marksheet of last Qualifying Exam (Final Year BSc Nursing/Post Basic BSc Nursing) (Mandatory)
    5. Provisional/Final Degree Certificate (Mandatory)
    6. Registration Certificate from Assam Nursing Council (Mandatory)
    7. Upload Appointment Letter (In case of in- service NHM Employee)
    8. NOC from respective Jt. DHS. (In case of in- service NHM Employee)
    9. Caste Certificate
    10. Experience Certificate
  4. Upload documents only in pdf.,.jpg or .png format.
  5. Then Click the Final Submit Button.
  6. Application process is not completed until go to the Final Page.
  7. In the Final stage you will get Registration No and your application for print will be available.
  8. Keep your registation no and password for future reference.
  9. Click the Print Application link to view and print the application.
  10. Take a print out of the application for future reference.
  11. In Case of multiple certificates use single pdf document including all certificates.
